TADQ's core service is in providing technical solutions to Client's requests. To do this, TADQ follows a 8-step process regarding its project requests, which conform to HACC standards. TADQ’s Occupational Therapist assesses each project request, and matches to a relevant Volunteer, where possible.

TADQ Technical Volunteers provide hundreds of effective project solutions on an annual basis, ranging from minor adaptations of aids through to larger items for schools.

How to request a project

TADQ takes requests from therapists and individuals, by phone, fax or email, to adapt or modify existing items, or to fully create new ones.
Requests can be for an individual or a group, and need to be made on the relevant form.
It is important to note for requests for individuals that one request per form needs to be completed and submitted.

If making a request by a School or Organisation we now have a form to make the information process a bit easier for you.
You can make multiple requests on the new form and each request will have an individual project number.
Please note: When you are requesting items for an organisation or school, you are required to provide a client name form the project, even if it is to be owned by the school or organisation. This is a requirement of the TADQ funding body.

Please provide as much detail and information as possible, including any drawings or professional information/advice to ensure a quick response to your projects needs.
(When sending photos, please email them to tadq@tadq.org.au with the clients name as a reference. If they are faxed you will be requested to email or post in to ensure we receive ledgible photos.)
